7) proxy for customs declaration (in triplicate with corporate seal and official seal)

8) customs declaration form (with special seal for customs declaration) I

Documents required for dangerous contract certificate (result sheet) (freight forwarding agent) :

1) packing performance sheet A, user: overseas factory is the exporter B. it can be issued by commodity inspection bureau of Shanghai or other places (commodity inspection bureau of the place where the packing plant is located)

2) IMI report of Shanghai commodity inspection bureau A, applicant and manufacturer: non-local factory

3) application for inspection of transportation and packaging of outbound goods (stamped with the official seal of the applicant at the upper right corner and printed)

4) the original record form for the appraisal of the use of packaging containers for dangerous goods exported by sea (Shanghai entry-exit inspection and quarantine bureau).

5) letter of proxy for inspection (stamped with official seal and legal person seal)

6) MSDS
